Position Description: The Transportation Resource Specialist assesses and addresses clients’ barriers to care, determines appropriate transportation resources available, and coordinates clients’ access to transportation services. The Transportation Resource Specialist also works as a part of an Interdisciplinary Health Team to coordinate transportation to medical and social service appointments.

•Schedule, Confirm and Pick up clients to en ensure they arrive on time for all ap appointments by planning and following the mo most efficient transportation route

•Ensure all clients have an up to date tr transportation waiver on file

•Collaborate with medical and social work st staff to determine urgency of transit needs

•Determine most effective transportation resource and provide linkage fo for transportation resources, including Wi Wiregrass Transit, Greyhound, Personal Ve Vehicle (gas cards), Medicaid Voucher

•Coordinate MAO Transit schedule with Co Community Outreach Social Worker for all ap appointments

•Report any vehicle mechanical problems to su supervisor and keep van exterior and in interior clean for passengers

•Comply with traffic regulations in order to op operate vehicle in a safe and courteous ma manner

•Accommodate clients with disabilities by lo loading and unloading wheelchairs, crutches, br braces and other medically necessary items

•Demonstrate and relay a skilled knowledge of of HIV / AIDS issues and their effects on in individuals, families, and special po populations

•Maintain required client records, document se services provided, and complete other re required paperwork in a timely manner

•Participate in continuous quality im improvement and outcome evaluation activities

•Provide individual and group educational sk skill building opportunities to foster ad adherence to medications, identify st strategies to improve adherence to health ro routines, communication with providers and ad additional issues to increase engagement in ca care and adherence to treatment

•Conducting informal assessments of client’s ne need for primary care/treatment and/or su supportive services, early in intervention/diagnosis information, peer ba based counseling, and general assistance in ac accessing appropriate services

•As with all other positions, during em emergencies, the Transportation Resource Sp Specialist is expected to fill in at any po position within the agency for which he / sh she is capable of performing

•Perform any other department / agency re related duties or special projects as di directed by supervisor

Qualifications: •Must be able to pass a drug test

•Must be 21 years of age

•Must confirm effective auto-insurance coverage and driver’s license if utilizing a v vehicle for work related reasons
