Position Description: This position will work toward the development of hospital-based secondary adult fall prevention system programs, and work on receiving notifiable patient information for older adults who have suffered fracture(s) due to an adult fall. Incumbent will follow on procedures to assure that appropriate preventive measures against future falls have been implemented. This position will provide program support by implementing and maintaining instructor training and certification of evidence-based fall prevention programs and developing and disseminating an effective referral system of prevention activities.

This position is a Pay Band 65.

Qualifications: Bachelor's Degree in Public Health or Education and two (2) years of experience in a health related field. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience.
