Position Description: The Community Medicine Program (CMP), a Health Care for the Homeless program, provides direct medical care, outreach, health screenings, health education, case management, crisis intervention and long- term counseling to some of New York’s most vulnerable residents at 11 shelters, drop- in centers and SRO hotels throughout New York City. The CMP team includes Physicians, Psychiatrists, Nurse Practitioners, Registered Nurses, Psychiatric Nurses, Social Workers, HIV Counselors, Outreach Workers/Navigators, and Substance Abuse Counselors.

As part of the CMP leadership team, the Manager will oversee daily operations of a robust and geographically dispersed system of health care for the homeless to ensure optimal operation of the practices.

Responsibilities include: • Provide oversight of staff, including ancillary and support. Coordinate with clinical leadership to ensure adequate staffing. Responsible for hiring staff and taking disciplinary action as needed. • Facilitates development and implementation of reengineering systems and new technologies to promote efficiencies. • Responsible for staff education and coordinating with clinical areas to promote patient-focused care, cultural competence and team building. • Ensure that all CMP procedures are in compliance with organization standards (e.g., safety and infection control). • Responsible for marketing, outreach, and health education activities and serving as the institutional representative at community events. • Prepares and reviews statistical reports required for internal and external reporting regulatory agencies. • Responsible for daily operations consistent with organizational goals and objectives. Collects data on, reviews and takes action on benchmark indicators (Quality of Services, Quality of Work Life, Quality of Care, Cost/Efficiency). • In collaboration with the clinical leadership, oversee budgetary and financial activities specific to operations. • Resolves patient complaints, monitors patient and staff satisfaction. • Design and organize educational programs for CMP and partner staff to promote patient care through improved communication and coordination. • Participate in preparation of grant proposals and negotiations for funding (new and additional) from grantors, contractors and sub-contracting agencies. Responsible for CMP grants and contract management. Participate in timely completion of required reports to funding agencies. • Participate in City, State and National Healthcare for the Homeless networks

Qualifications: ? Bachelor’s Degree. Master’s Degree in Public Health, Business Administration, Social Work, or related field preferred. ? 5+ year’s progressive experience in Ambulatory Care, managed care, health administration or related field (Managed Care experience preferred. 3+ experience working with individuals who are homeless. • Excellent oral/written communication skills; Good computer skills. • Strong knowledge and experience with Managed Care/health insurance procedures and systems; • Knowledge of local, state and federal health care regulatory, licensing and accreditation procedures and requirements. • Extensive knowledge of the unique needs of individuals who are homeless.
