Position Description: Job Title: Coordinated Intake and Referral (CI&R) Manager Department: Healthy Start Location: Gainesville, FL Posting Closes: August 7, 2017 Submit Resume: General Description This position is responsible for all aspects of the Coordinated Intake and Referral (CI&R) system for pregnant women and families with young children. The primary duties of this position are to work collaboratively with participating agency representatives to develop, implement and provide oversight of the CI&R system. The CI&R system ensures that families are obtaining home visiting services that are most appropriate for their needs; creates one point of services; and promotes collaboration between all partners providing services to pregnant women and families with young children. Knowledge, Skills and Abilities: • Knowledge of community resources and home visiting programs for pregnant women and young children • Knowledge of assessment tools and processes • Knowledge of State and Federal laws and regulations • Knowledge of Microsoft applications • Excellent verbal and written communication skills • Excellent time management, organizational, and coordination skills • Excellent problem solving skills • Excellent critical thinking skills • Resourceful and willing to take initiative to seek information as needed • Motivated and self-directed • Able to adapt to a changing environment • Able to handle sensitive and confidential information in a professional manner • Able to work independently with minimal supervision • Able to effectively prioritize assignments and meet deadlines • Able to establish and maintain positive relationships Essential Duties and Responsibilities: • Establishment of the CI&R system o Establish local CI&R teams comprised of maternal, infant, and child health home visiting programs, services providers and screening infrastructure o Act as the main point of contact between the Healthy Start coalitions and all other participating agencies of the CI&R system o Develop memorandums of understanding (MOU) with community agencies o Coordinate all CI&R project team meetings including securing meeting space, communicating times and dates to team members, recording notes, and following up on action items o Develop and test decision trees and algorithms using the Healthy Start Prenatal Risk Screen o Develop, test and implement a centralized process to track CI&R clients o Ensure that the program is in compliance with internal policies, procedures, and other applicable licensing, regulatory and contractual standards o Work with the Program Director and Business Operations Specialist to ensure that all reporting deadlines are met and invoices are submitted to grant administrators • Provide oversight for the staff and operations of the CI&R system o Conduct employee interviews with Program Director as needed o Orient new staff to policies, procedures, and program expectations/requirements o Provide leadership, guidance, support and supervision to program staff o Assign and review caseload with staff to ensure cases are in compliance o Ensure screens and referrals are reviewed and assigned in a timely manner o Ensure and monitor completion of all required records, reports, and documentation in order to comply with organizational standards o Investigate and respond to clients regarding complaints and/or concerns o Develop and maintain a good working knowledge of the Healthy Start Standards and Guidelines, Well Family System and community resources o Oversee data entry, data collection and compilation of all programmatic/statistical information for reporting to and from providers and contract partners o Review and ensure compliance with productivity standards • Participate in public relations and community education efforts • Participate in required program trainings and staff meetings • Perform other duties as assigned Education and/or Experience Master's degree in public health, health administration, social work or similar field; or bachelor’s degree in related field and two to four years related experience and/or training; or equivalent combination of education and experience.
