Position Description: Coordination of initiatives such as DSRIP, Care Transitions Network, review of metrics and reporting such as ED visits, Hospitalizations, time from referral to assessment, and would involve using PSYKES and our EHR data. Attend meetings and coordinate with stakeholders.

Qualifications: Bachelors or Masters in Public Health or Behavioral Science. Ability to work with well data to create and analyze reports. Ability to coordinate initiatives to assist in translation from initiatives to outcomes. Familiarity with Electronic Health records, reporting, and state health databases.

Organization Description: Bleuler Psychotherapy Center is a NFP Behavioral Health Clinic serving the community for over 65 years. We operate Article 31 and 32 therapy and substance abuse clinics and provide services to an ADP of 2000 accounting for over 60K services per year. We employ over 80 clinical staff members (Psychologists, Psychiatrists, Social Workers, Mental Health Counselors) who speak multiple languages and represent the diversity of our community. We are currently evolving towards outcome and evidence based services and a Value Based Payment structure.

Additional Information: This is a new position however it is fully approved and invested in for the long term. It involves being the lead for behavioral health transition to outcome based and value based services with a focus on the integration of hbehavioral and physical health. An interest in public health and fluidity with data and epidemioloigical issues is preferred. We are a warm and inviting work place with full benefits and a fun location just outside of Manhattan.
