Position Description: The primary job function is to coordinate a Rapid Entry Clinic for HIV-infected adults. The Linkage Coordinator is the backbone of the Rapid Entry Clinic, and will identify barriers to care, refer to other services (e.g., case management, mental health and/or substance abuse treatment), refer and link clients to long-term HIV care providers, and monitor appointments and health outcomes.

Essential Job Functions: 1) Coordinate the Rapid Entry Clinic for clients newly HIV diagnosed or those re-engaging into care 2) Assess potential barriers to care and the impact of HIV stigma with newly HIV diagnosed and re-engaging clients 3) Screen clients for mental health, case management, and substance abuse services utilizing the Atlanta EMA screening tool, and refer for further assessment and services as needed 4) Schedule medical appointments with the rapid entry clinic providers 5) Enter clients into the electronic medical record (EMR) 6) Coordinate with the clinical team to ensure new patient laboratory tests are collected 7) Check Ryan White eligibility by collecting eligibility documentation 8) Develop a sustainable HIV care plan with each client that focuses on removing barriers to care, determining benefits, and linking to a long-term care provider 9) Facilitate linkage to a long-term care provider by reviewing provider options with the client and making appropriate referrals 10) Monitor show rates for all appointments including follow-up with the long-term care provider to verify that the initial medical visit occurred 11) Contact pharmacy to check that antiretroviral prescriptions were filled 12) Monitor health outcomes of Rapid Entry Clinic clients and assist with data collection and reporting

Qualifications: Required Knowledge, Skills and Personal Qualifications: 1) Bachelor’s degree in social work or related human services or healthcare field and a minimum of two years prior experience in a healthcare setting 2) Experienced in working with indigent and culturally diverse populations 3) Electronic Medical Record system knowledge preferred 4) Ability to work without close supervision 5) Utilizes good management techniques and time management 6) Proficiency in word processing and spreadsheet database

Preferred Qualifications: 1) Master’s degree in social work or related field 2) Experience working with HIV-infected clients 3) Experience with project management including: developing, implementing and evaluating health care programs
