The Manager (or Director) must have an understanding of the general goals and objectives of population health as well as a broad understanding of data, workflows and systems, which will be leveraged for clinical and process improvement, for helping our members achieve health and wellness and for reducing the total cost of health care within our network. The Manager/Director will work closely with physicians and management in the development of clinical and care management guidelines, protocols and other tools to advance a scalable and systematic approach. The Manager/Director will compile, synthesize, manipulate, and analyze data from a wide range of sources (e.g., claims, health information exchange, physician practices, etc.). The Manager/Director will format the data in a high-impact, visually appealing, easy to understand manner to guide network physicians, care coordinators and management in establishing benchmarks, measuring quality and process- related outcomes, identifying areas for continuous improvement, and assessing the impact of various clinical and/or programmatic interventions. The Manager/Director will work with network physicians and will engage directly with network members (patients) by phone to help members navigate the health care system, specifically supporting members during “transitions of care” (e.g., going from hospital to home) and to help members achieve better health and wellness as it relates to chronic diseases (e.g., asthma, diabetes, hypertension, etc.) or other complex health care needs. A successful candidate must be passionate about improving the health of populations, specifically using data for problem solving; He/she will be self-driven, able to lead a project from end to end, and will be an effective and likeable communicator and collaborator across multiple, cross- functional teams and most importantly, with our members. He/she will support the Monument Health mission of a “healthier Mesa County” and operate per Monument Health core values.

Qualifications: • Bachelor’s degree in healthcare or quantitative field (e.g., math, computer science, etc.) required; Graduate degree (MPH, MHA or MBA) strongly preferred • Minimum of three (3) years working in a healthcare or health plan setting required; Experience in quality or process improvement, process engineering, data/analytics and/or member services preferred • Intermediate to advanced Excel skills required. (Please note, Excel skills will be assessed during the interview process) • Intermediate skills with other Microsoft products (e.g., Word, Power Point, etc.) required • Experience with other data warehousing activities, report development and visualization applications (e.g., Tableau) or statistical software applications (e.g., SPSS, etc.) helpful • Experience working with members (patients), physicians, other health care providers, administrators and people in general is a must • Demonstrated ability to lead a project from end to end (e.g., develop a project plan, establish metrics, plan meetings with clear and constructive objectives, facilitate meetings with physicians, management, collaborators, other professionals, etc.) • Ability to independently exercise good judgment, identify issues and trouble- shoot (i.e. able to think on feet and act quickly) • Experience having managed multiple projects at once and able to balance multiple stakeholders’ opinions and priorities • Excellent written and verbal communication skills (i.e. able develop a clear and coherent slide deck and facilitate robust discussion with physicians, management or other professionals with clarity, confidence, and credibility)

Organization Description: Monument Health, LLC is a start-up organization on Colorado’s Western Slope that facilitates a network (or team) of providers and payors who work with our members (patients) and with each other to deliver better care at a lower cost. We’re looking for a Manger (or Director) of Population Health to help lead the organization.
