Position Description: The Quality Specialist will have primary responsibility for implementing systematic, practice-facing continuous quality improvement efforts to improve processes and outcomes within the collaborative care models at 17 NYC Health + Hospitals facilities (hospital ambulatory clinics and community health centers). • Manage a caseload comprised of a subset of NYC H+H’s outpatient practices (adult, adolescent, OB) in the engagement of quality improvement initiatives using practice facilitation strategies. • Conduct on site visits to provide technical assistance to primary care and collaborative care teams – including PCAs, nurses, physicians, psychiatrists and social workers – improving processes and patient outcomes related to the collaborative care model. • Provide support for workflow redesign to improve efficiency and effectiveness for identifying and treating mild to moderate mental health conditions in the primary care setting. • Provide coaching on the utilization of data to drive continuous quality improvement. • Serve as a liaison between facility staff and central office collaborative care program staff to provide feedback on various QI efforts. • Deliver QI related presentations and trainings to facility and central office audiences to support capacity building for sustainable continuous quality improvement within the practice.

Qualifications: Minimum Qualifications • The individual for this position will will have a bachelor’s degree, master’s degree preferred • Two years plus relevant work experience in practice/quality improvement • Ability to travel within the 5 boroughs of NYC • Advanced skills in data management as well as the ability to present data to groups of individuals

Preferred • Experience completing or leading quality improvement initiatives in primary care practices • Knowledge of primary care behavioral health integration initiatives • Ability to work collaboratively with health and mental health professionals • Strong computer skills • Experience in health care delivery systems redesign to support collaborative care and primary care, behavioral health integration initiatives. • Excellent ability to develop relationships with diverse stakeholders. • Excellent time management and communication skills.

Reporting to the Program Manager for Collaborative Care, the Collaborative Care, Quality Specialist will provide support for NYC H+H collaborative care initiatives to improve chronic illness and behavioral health outcomes in our ambulatory clinics, with particular focus on depression, and expanding to other conditions such as anxiety and substance use.
